How can I get copies of a birth or death certificate?

Requirements for birth, death, and marriage certificates change from time to time. The requirements imposed by the State of Georgia for birth and death certificates may be found at the State maintained web location: dph.ga.gov

Requests for birth certificates, death certificates, and marriage certificates can be made in writing or in person. Please see the Forms section of our website for the relevant request forms. What is the fee for a copy of a birth or death certificate?

$25.00 for the first copy, $5.00 for each additional copy purchased at the same time for the same person. All copies are certified.

Who may get certified copies of a birth or death certificate?

Only those individuals with a direct and tangible interest, primary immediate family members, or legal representatives of the family may request a certified copy of a birth or death certificate.  Valid identification must be provided by the requesting party.
